John Milton is up against the clock: Jonah King, the leader of a Satanic cult, has murdered Milton's daughter and kidnapped her baby. In three days, King and his followers will sacrifice the child at midnight. Milton picks up the trail in Oklahoma as well as rescuing a waitress named Piper from her brutal, two-timing fiancé. There are odd things about Milton: his driver's license is out of date, he has a very strange gun, and he's being pursued by a man in a suit who carries FBI ID and calls himself the Accountant. Piper, who's lived a life on the sidelines, has to piece things together on the fly as they close in on King.

A man gropes his employee in public by trapping her with his body while putting his hand on her stomach. She responds by grabbing him by his genitals and slapping him.

A woman drags a naked woman by her hair. She then pushes and punches her so hard she falls to the ground.

There are several gunfights throughout the movie. Some involve blood, some don't.

A young woman gets into fistfights in a few instances. Her face and body are bruised.

In a flashback, a man pushes a woman to a wall, holds a straight razor near her neck, and cuts it. We only see him draw the razor quickly left to right, and a large blood streak splattering on the wall. Then, we see his face in close up as he licks blood from his fingers.

A man pins another man's shoulder with a table leg. He then drives the table leg toward the other man (offscreen). We later see it piercied in his eye socket, with a lot of blood around it; the man is dead.

A man shoots another man and blows his hand off, and a lot of blood spatters; then the shooter fires into another man's leg, and a third man is shot in the chest. Another man crawls on the floor as the opponent beats his back with a human femur: the other man picks up a rifle, fires and blows his enemy into the sky; his body lights up and disintegrates, and his reddened skull falls to the ground.

A man accidentally buries a machete into his own skull and a woman buries a hoe into the back of the head of another man.

A man shoots two people in the forehead and we see a dot of blood as they fall offscreen.

A man suffers an open wound to the knee; another man takes a pocketknife and stabs him repeatedly in the knee as the wounded man screams.

A woman gets into a physical fight with her kidnapper. He forcefully kisses and chokes her. She stabs him in the chest with a pentagram necklace. We see him bleeding. She escapes the man by throwing herself out of the window of a moving van and onto an oncoming car. The man tries to shoot her.

A man knocks a woman to the ground, and a second man kicks the first several times, knocks him down, and steps on his face, keeping him down.

A clothed man simultaneously smokes, drinks, has sex with a nude woman and participates in a long gunfight with several people; he wounds all of the opponents, drops the unharmed woman and leaves.

Many gun and rifle barrels aim into the audience in 3D, with bullets discharging in flames. A man and a woman encounter a roadblock and a trooper tells about 50 troopers with rifles to aim at their heads.

A man holds a knife to the throat of a woman.

An FBI agent orders two police officers to find a man and "shoot to kill."

Men and women gather in an empty bar and a man tells them to go kill another man in one of the guest rooms.

A TV reporter announces that a religious sect is responsible for a string of murders.
