This is actually pretty mild for an R rated movie.

Teenagers drink and a man smokes a cigarette.

All of the scenes involving decapitation are pretty intense.

The Horseman may frighten some towards the end when he starts to get his head back.

Most of the movie is about curses and junk which may frighten lots of people.

Honestly this movie should have a PG-13 rating, but since it is SyFy I guess they would put it as R for the fun of it.
