A woman opens a door, causing an oxygen tank to explode. This kills both her and a man that is in the room.
A woman drives an ambulance into a lake in order to drown herself. A man tries to rescue her by breaking the windshield with his gun.
We then see parts of that accident occur again (from a different perspective) and then a truck smashes into a smaller truck, causing the latter to explode, killing the three people inside it (we later see that on TV footage).

While a little tame compared to the first, this is still a very graphic movie that was hard R (UK-equivalent 18) originally and now moderate R (UK-equivalent 15) only on appeal. Lots of graphic bloodshed, accidents, and very strong horror/thriller violence.
