Hellroller (1992)

Eugene's aunt tells him about his real mom giving birth to him as a result of being raped by Siamese twins.

There are two long scenes of female stripping and constant nudity. Breasts are clearly visible in both scenes.

There are several brutal killings towards women, which many of these killings tend to be heavily implied. Sometimes you don't see anything happen, until you see gory aftermath of said killings.

A man gets killed during a weight training, by getting a heavy barbell dropped over him. We don't really get to see the actual impact.

Two women get impaled in a row.

Eugene's backstory about his birth (see Sex & Nudity) is bound to be disturbing.

Despite most killings being heavily implied, the aftermath might make these scenes disturbing/intense.
