When Hogan and Sara arrive at the Juarista camp to meet Beltron, a Mexican woman is breast feeding. She pulls the child away and her nipple is clearly visible.

Opening scene shows "Sister Sara" in compromising position with clothes half off and being harassed by a gang in the desert side of her breast and butt can be seen, implying attempted rape. Later, discussion occurs between Eastwood's character and Sara about her wanting to make love to a man. This scene lasts about 4-5 seconds.
