This is a 1929 film, so obviously the first one is done with special effects. It's extremely graphic nonetheless:

A woman's eye is slit open with a straight razor, causing liquid to ooze out. They actually used a cow carcass for the effect, so it is real to some degree.

Two dead donkeys are seen, one is dripping blood from its nose, and has maggots crawling in the eye sockets.

When the woman's eye is slit, it is very realistic. Although the film is art, many will find the random disturbing images unnerving.
