Mild
Male characters are shown shirtless sometimes, not often. If you let your kids play Street Fighters and Tekken then this should be okay, it's literally no different.
Sanji tends to flirt with women a lot kind of like Brock from Pokémon but it's mainly played for laughs. The top of women's clothed boobs are shown sometimes.
The series has no sex, however there is some nudity. Examples; Women are shown naked, but sensitive parts are covered by leaves, bushes, fog, etc. Side boobs (which we see in kid cartoons anyway) are the only thing shown. And in rare cases if it's a front camera angle, their hair covers their nipples so we only see the outline of their boobs.
Women are in bikinis sometimes and their side boobs and the top of their boobs are shown. (The nipple isn't visible)
Sometimes a girl hugs another character and the camera angle shows her boobs next to the characters face (The character's reaction is always annoyed)
There's a scene where a woman is sitting in the bathtub with her daughter, and her daughter puts her two hands on her boobs (censoring her boobs and nipples from the viewers) all we see is her side boob.
In the "Island of Women" season lots of women in bikinis, some outfits show a decent amount of cleavage.
There is some suggestive humor throughout, but it is rarely and mild.
Sensitive parts like nipples, butt, and vagina are censored by surrounding objects, the side of boobs are the only thing shown.

Characters are sometimes seen swimming in bathing suits.
Nami wears a swim suit as casual outfits because she's a pirate and she goes swimming a lot.

The series doesn't focus on fatal injuries but usually there is moderate blood shown in most of the fights. Still not very gruesome
Some of the fight sequences include small amounts of blood (especially during the sword fights), characters are often injured, but death isn't common, and when it does happen, it's never shown on screen.
A wide range of violence from slapstick comedy antics to intense fights that can get brutal.
In a flashback early in the series, a pirate that is friends with the main character gets a gun pointed at his head by a bandit. One of the pirate's men hits the bandit and saves the pirate.
Luffy got his chest burned during a war while he was unconscious, leaving a huge X scar on his chest. it healed pretty quickly though.
There is one character shown with a peg leg and says he lost it saving a kid.
The entire series is violent and can get pretty bloody. However, it doesn't linger too much on injury detail, so it's not that bad.
There are many scene of violence and blood throughout in every episode of the show. Sometimes it for humor purposes, and sometimes it's more serious and not humorous. Fights are taken place in a lot of episodes too.
There's plenty of cartoonish slapstick whenever the characters get into comical situations.
Over the course of 1100+ episodes, there are many fights that show characters getting physically hurt in some form or another, complete with bones breaking and blood flying everywhere.
Moderate violence, with some bloody violence, and even rare animated gore, but some of it is humourous, but most of it is serious and action packed.
Overall, the violence and blood/gore doesn't mean to be extreme. But there are a few arcs have more violent and bloody. Marineford Arc and Land of Wano Arc are being a good example.
Characters get shot at by cannons and guns at times.
Some violence like explosions, bloody violence, weapons uses, special powers, martial arts cartoon violence, yelling, and slapstick.
Moderate overall, I wouldn't say there's anything severe.
The show has some evil characters throughout the series. Most of them learn their lesson though and become good hearted people later on in the show.
One of the main characters (Usopp) gets beaten badly. He survives all of these attacks but is a total coward (which is due to his insecurities and his tragic past), which greatly amplifies the violence that is dealt with him. In one scene, he gets hit straight in the face by a 4-ton baseball bat. We see his broken nose later on. In another scene, he gets brutally beaten and stomped on by some men. These are of course one of the many beating scenes of this particular character, but he survives.
A pirate gets kicked out of a window when he accuses a pirate of cheating in a card game.
The main character Luffy suffers incredible physical and mental pain throughout the series.
The show talks about how Gol D. Roger got executed and how his last words were what sparked the new pirate era.
2 pirates die on screen in the series, the rest of them get captured. (One dies by old age, while the other, dies saving Luffy from a Navy Admiral. He gets hit with magma and no blood is shown.)
Profanity

There is some very mild language and it'sinfrequently.
Multiple with various of childish name calling words, "stupid", "jerk", "idiot", "fool", "brat", "dumb", "moron" etc. A running gag involve Luffy has funny nickname to other characters sometimes (For example, Luffy call Buggy a "Big red nose" due to the characters design). Played for laughs.
Many mild curse words ("damn", "hell", "crap" and "piss")/PG language are spread out of the series throughout in both the English subtitles and the Dubbed version.
Damn is used a few times but the show is so long it's not frequent .

During some of their lighter moments, some characters are shown drinking alcohol like rum since they are pirates but no one ever gets drunk.

Since the characters are (mostly) pirates of the show, both alcohol and smoking occur very frequently. Many of the main characters smoke cigarette/cigars and drink alcohol.
Alcohol (especially beer and rum) is the typical thing with pirates, so expect to see a lot of it.
Sanji (one of the main characters) is a smoker and is always seen with a cigarette in his mouth.
One character, Smoker, is always shown smoking two cigars at once. He has a bandolier filled with cigars.

Frightening & Intense Scenes

Despite One Piece being a fairly light hearted series, and the violence not being too extreme throughout, there are still some scenes which can be disturbing throughout the series. These however are normally not presented in an overly violent matter but it doesn't take away from the impact.
Many of the backstories throughout are disturbing, emotional, upsetting, and intense.
There are many, MANY emotional and sad scenes throughout the series. Many minor and major characters died in very tragic events, and some characters have devastating emotional breakdowns that result from their parents/siblings/friends dying. Although the series is lighthearted and happy-go-lucky, it has a very emotionally intense tone, but the humorous scenes can help recover from the emotional/upsetting stuff.
There are many scenes of tension including perilous situations, destruction, national disaster, war, etc.
Different forms of racism are occasionally present in the show, especially the land of wano arc.
The main cast, while pirates, only resort to violence if there's no other alternative, and there are moments when the more aggressive characters are shown that violence will only make matters worse.
While the design of this anime looks like a cartoon (what with it being very colorful, bright, cartoonish and lighthearted), but don't let that fool you, because it has a lot of very dark, mature and extremely emotional stuff.
Out of the big 3 anime of Shonen Jump (others being Naruto and Bleach), One Piece is the longest running and lighthearted series, with 1100+ episodes (due to long arcs).
There's lots of very mature themes in the series, though the cartoonish aesthetic and mostly light-hearted mood of the series balance them out.
As the series progresses, it gets darker, intense, emotional and exciting in much later on.
Luffy and friends get involved in a lot of problems during the adventure to find a legendary treasure, "One Piece".
Mild/Severe for neuordivergent viewers.
The fellas who have Devil Fruit powers are also unable to swim. Which can be a risk for many like Luffy, Buggy, et al.
This show can get fairly violent at times. There are several instances of slapstick humor, but others bloody violence can get intense for some viewers. Characters use swords, axes, knives, cannon, saws, guns, and explosives.
For older tweens (11-13) and up.
Moderate overall. Although sometimes it can get severe.
Some villains like Kuro, Bluejam, Hody Jones, Doflamingo and the Celestial Dragons are complete monsters who are devoid of empathy and sadistically kill and torment anyone in their way.
Though nothing pops out or scares you, some scenes in which people are on the brink of death or in an intense situation might scare some viewers. The lighthearted and very funny scenes can help recover from the intense stuff.
Lots of scenes where main characters come very close to death.
Emotional and intense scenes frequently occur throughout the show. And many of the characters' backstories are quite tragic.
This is a surprisingly dark and emotional anime overall, even if it's for the most part lighthearted and happy-go-lucky.
